Most modern immunoassay platforms detect the analyte using a biotin–streptavidin capture step, so biotin circulating in the patient’s sample competes with the assay’s own biotinylated reagent. The result is a wrong number rather than a flagged error: falsely low on sandwich assays such as TSH, PTH, ferritin, β-hCG and cardiac troponin, and falsely high on competitive assays such as free T4, free T3, testosterone, oestradiol and cortisol. Singapore’s Health Sciences Authority has reported interference from oral products containing 150 mcg or more of biotin per dose unit — far below the 5,000–10,000 mcg in hair, skin and nail supplements. The US FDA issued a safety communication on this in November 2017 and has described a reported patient death following an incorrect troponin result. The fix is not to abandon biotin. It is to know the number the patient is actually taking, formulate it deliberately, and — where the goal is hair and skin support rather than correcting a true deficiency — move the actives to a compounded topical so the serum is never loaded at all. How the interference works, and which way each test moves
The mechanism is not a quirk of one manufacturer but a design choice shared across the major analyser platforms. Biotin binds streptavidin with one of the strongest non-covalent affinities in biology, which makes the pair an excellent way to anchor an antibody complex to a solid phase. Exogenous biotin in the patient’s serum saturates that anchor and displaces the intended complex, so the analyser reads a weaker signal than it should. What that weaker signal means depends entirely on the assay format — and this is the part that catches clinicians out.

Two caveats matter before anyone builds a rule from that table. Susceptibility varies by platform and reagent lot, because manufacturers differ in reagent load and wash chemistry — some assays tolerate concentrations that break others. And the interference is invisible: no haemolysis flag, no delta check, no QC failure. The number arrives plausible and wrong.
The signature pattern: a thyroid panel that looks like Graves’ disease
Because TSH is a sandwich assay and free T4 and free T3 are competitive ones, biotin pushes them in opposite directions at the same time: a suppressed TSH alongside elevated free T4 and free T3 — the textbook picture of thyrotoxicosis, in a patient who is euthyroid.
The published case literature describes exactly this as factitious or “biotin-induced” Graves’ disease. Worse, the confirmatory test can fail in the same direction: TSH-receptor antibody assays have been reported to read falsely positive in the presence of biotin, so the follow-up investigation appears to corroborate the diagnosis rather than refute it. Reported consequences include unnecessary antithyroid medication and imaging before anyone thought to ask about a hair supplement.

The dose problem: 30 mcg versus 10,000 mcg
Adequate intake for adults is commonly cited at around 30 mcg a day, and frank deficiency is described in the nutrition literature as rare in people eating a varied diet. Retail hair, skin and nail products are typically formulated at 5,000 to 10,000 mcg; the FDA has noted products reaching 20 mg — more than 650 times the recommended allowance.
Set that against the HSA figure of 150 mcg per dose unit and the arithmetic is stark: a standard beauty supplement can sit thirty to sixty times above the level at which interference has been reported. This is not a fringe overdose scenario. It is the default pack on the shelf.
What the evidence actually supports for hair and nails
It is worth being straight about why the dose is so high: nobody chose it from an efficacy study. A widely cited 2018 review concluded there was insufficient high-quality evidence to recommend biotin supplementation in healthy people, and subsequent reviews of biotin monotherapy have not found consistent benefit on objective hair-growth outcomes; the strongest controlled study found no difference from placebo. Where the literature does support biotin is narrower — inherited and acquired biotin deficiency states, brittle nail syndrome, and uncombable hair syndrome.
So the honest position for a prescriber is that a megadose is being given to a largely non-deficient population, on evidence that studies suggest is weak for that population, with a documented downside in the clinic’s own laboratory results. That is an unusually clean case for formulating deliberately instead of buying off a shelf.
The washout is a prescribing problem, not a laboratory problem
Biotin has a short plasma half-life in normal renal function, so interference is transient. Laboratory and specialist guidance commonly suggests stopping standard-dose supplements for 48 to 72 hours before testing, with longer intervals — in the region of three to seven days, and up to one to two weeks for very high or therapeutic doses — suggested where intake has been substantial. Impaired renal clearance prolongs it.
This almost never fails at the lab. It fails because nobody knows the dose. A patient on “a hair supplement” cannot tell you whether that is 300 mcg or 10,000 mcg, multivitamins are rarely volunteered as supplements at all, and a combination product may carry biotin the patient has never noticed. A washout instruction is only as good as the number it is calculated from.
What a compounding pharmacy can change
A prescription-led formulation does what a retail pack structurally cannot.
- A known number. The prescriber specifies the biotin content, and it appears on the label of that patient’s preparation. The washout window can then be calculated rather than guessed.
- A dose chosen for the indication. Where the intent is nutritional adequacy rather than a megadose, biotin can be formulated at a physiologic level — and, where the prescriber wishes, held below the reported interference threshold, so routine monitoring is not disturbed at all.
- Omission as a formulation decision. A hair or skin support formula can simply exclude biotin, which a fixed retail combination cannot. Everything else the prescriber wants stays; the one ingredient corrupting the panel leaves.

For most patients taking biotin in a Malaysian aesthetic or hair clinic, the objective is local — the follicle and the skin — and the systemic exposure is an unwanted by-product that happens to be the thing corrupting the labs. Removing it is not a compromise; it is the more targeted option.
