Short answer: Lactoferrin, casein glycomacropeptide (CGMP) and alpha-lactalbumin are the three most interesting minor protein fractions in milk — and they cannot honestly share one capsule. Lactoferrin works at milligrams (100–300 mg/day in the clinical trials); alpha-lactalbumin and CGMP were studied at grams (20–30 g/day). That 100-fold gap, plus a charge incompatibility that makes cationic lactoferrin precipitate with the two anionic fractions in solution, dictates the design: an acid-protected lactoferrin capsule, and a separate evening oral liquid carrying the gram-dose fractions. In Malaysia, Lynnity prepares both only on a doctor’s prescription, compounded under Good Compounding Practice (GCP).

Three fractions, three different jobs

Bovine milk protein is ~80% casein and ~20% whey, but the pharmacological interest sits in the minor fractions. Lactoferrin is an ~80 kDa glycoprotein whose two lobes each grip a ferric ion — its biology tracks its iron-saturation state, a point we covered in detail in our compounded lactoferrin guide. CGMP is the 64-amino-acid tail of κ-casein released during cheesemaking: heavily sialylated, extremely soluble, acid-stable — and the only intact dietary protein containing zero phenylalanine, which is why it anchors modern phenylketonuria medical foods. Alpha-lactalbumin is a small 14 kDa whey protein and the richest natural source of tryptophan (~4.8 g per 100 g protein) — the property behind its sleep-and-alertness trial record.

The evidence a prescriber would actually cite

Lactoferrin carries the deepest file: randomised trials in recurrent respiratory infections in young children (a 2024 RCT reported roughly half the episodes versus placebo), meta-analysis in pregnancy iron-deficiency anaemia showing haemoglobin responses comparable to ferrous sulfate with fewer gastrointestinal complaints, and a stable-isotope crossover study showing that the protein’s iron-saturation state changes iron absorption itself (Figure 2A). It also carries the field’s most instructive null — ELFIN, in The Lancet 2019, found no benefit in 2,203 very-preterm infants.

Alpha-lactalbumin has a genuinely specific mechanism: taken in the evening, 20 g raised the plasma tryptophan-to-large-neutral-amino-acid ratio by 130%, and in double-blind conditions improved morning alertness and sustained attention, with the clearest behavioural benefit in poor sleepers (Markus et al., American Journal of Clinical Nutrition 2005). A 2025 randomised placebo-controlled study in trained adults with sleep difficulties reported improved sleep architecture and faster morning reaction times. A 2024 systematic review is appropriately cautious: promising, modest, not a sedative.

CGMP deserves the most honest framing on this page. A rigorous Br J Nutr 2020 trial gave healthy adults 25 g/day and found no measurable change in satiety, microbiota composition or inflammatory markers. A small ulcerative-colitis pilot (30 g/day, adjunct) is interesting but preliminary. Where CGMP is genuinely irreplaceable is phenylketonuria — gram doses of a palatable, phenylalanine-free intact protein. In a combined preparation at single-gram amounts, its role is functional (solubility, mouthfeel, sialic-acid content), and we say so plainly rather than borrow claims it has not earned.

Why one capsule cannot hold all three

Run the arithmetic. The trial-anchored daily amounts are roughly 200 mg of lactoferrin plus 20 g of alpha-lactalbumin plus, where used at its studied dose, 25 g of CGMP. A size-00 capsule holds ~700 mg of protein powder — the gram-dose fractions alone would demand more than fifty capsules a day. Any single-capsule product listing all three names is, necessarily, carrying token amounts of the gram-dose fractions. That is a labelling observation, not a scandal — but a prescriber should recognise it.

The chemistry of putting them together

The second constraint is charge. Lactoferrin is strongly cationic at physiological pH (isoelectric point ≈ 8.7); CGMP is strongly anionic (pI < 3.8) and alpha-lactalbumin acidic (pI ≈ 4.2–4.5). Mixed in solution, they form electrostatic complexes — visible haze, sedimentation, and a hero protein tied up before it is swallowed. This is well-described protein chemistry, and it is why a serious preparation keeps lactoferrin in its own dry, acid-protected capsule and the gram-dose fractions in a separate liquid. Three further rules follow from the same chemistry: no iron salts alongside lactoferrin (loading the protein with iron changes its function); no heat at any point (alpha-lactalbumin denatures and Maillard browning begins); and no acidic or tannin-rich flavourings in the liquid (the pH 4–5 zone is alpha-lactalbumin’s precipitation window, and polyphenols precipitate proteins).

How a compounding pharmacy prepares them

In practice the prescription becomes two preparations. The lactoferrin capsule: an acid-resistant capsule at the prescriber’s chosen dose, from a certificate-of-analysis-verified raw material whose purity and iron-saturation state are documented — the specification a retail label never shows. The evening liquid: alpha-lactalbumin at a trial-consistent dose with CGMP as the functional partner, cold-dispersed in purified water, vanilla-flavoured, sweetened without reducing sugars, dispensed refrigerated with a short beyond-use date in line with our beyond-use dating practice. Nothing here is exotic — it is simply protein chemistry respected, which is what customised formulation work is for.

Safety — the screens that come first

All three fractions are bovine milk proteins: cow’s-milk-protein allergy is an absolute contraindication, screened before anything is compounded. Isolates are low in lactose, but severe lactose intolerance deserves a conversation. Iron status matters in context — lactoferrin questions often travel with personalised iron work, and the two are deliberately separated in time. Pregnancy and paediatric use belong under direct medical supervision. And the standing rule of every Lynnity preparation applies: supplements included, everything requires a prescription from a registered doctor. No preparation here treats, cures or prevents any disease; the evidence above is presented so that a prescriber can judge it.

Frequently asked questions

Can lactoferrin, CGMP and alpha-lactalbumin be combined in one capsule?

Not at the doses the clinical trials used. Lactoferrin is a milligram-dose active; alpha-lactalbumin and CGMP were studied at 20–30 g/day — dozens of capsules’ worth. They are compounded as two preparations: a capsule and a separate liquid.

Why can’t lactoferrin simply be added to the liquid?

Charge. Lactoferrin is cationic; CGMP and alpha-lactalbumin are anionic at drink pH. In solution they form complexes that cloud the liquid and bind the lactoferrin. It stays in its own acid-protected capsule.

Does CGMP “work” at a few grams?

Honestly: unproven. The best trial in healthy adults used 25 g/day and found no measurable effect. At small doses CGMP’s role is functional — solubility and palatability. Its irreplaceable clinical role is phenylalanine-free protein for phenylketonuria, at gram doses, under specialist care.

When is the alpha-lactalbumin liquid taken?

In the evening, matching the trials — the tryptophan-ratio rise precedes sleep, and the measured benefits (morning alertness, sleep architecture) came from evening dosing of about 20 g.

Who should not take these preparations?

Anyone with cow’s-milk-protein allergy, and anyone without a prescription — every Lynnity preparation, supplements included, is prescription-only. Severe lactose intolerance, pregnancy and paediatric use need a doctor’s individual judgement.

How is the liquid stored?

Refrigerated, with a short beyond-use date set at compounding. A near-neutral protein liquid is not a shelf product — freshness is part of the preparation.
